How does a Special Needs Plan work?
Medicare Advantage Special Needs Plans (SNPs) offer distinctive benefits, including mandatory prescription drug coverage. Furthermore, SNPs provide coordination services to help individuals manage and receive the care they need.
With your SNP, you are assigned a network of medical providers. Depending on your policy, you may also receive coverage for out-of-network care. Your agent can provide specific details about your coverage.
Enrollment in a Medicare SNP is available at any time. Generally, you must have a primary care provider or a designated care coordinator to assist you. Additionally, beyond basic preventative services, a referral is typically required to see a specialist with a Medicare SNP

· You already have both Medicare and Medicaid

(I-SNP)
Institutional Special Needs Plan (I-SNP)
· You live in an institution such as a:
- – Nursing home
- – Skilled nursing facility
- – Long term care skilled nursing facility
- – Intermediate care facility
- – Inpatient psychiatric facility
· You require nursing care at home
